School Year Requirement

Proof of current immunizations is required for school attendance under Arizona State Law A.R.S. 36-671, 15-871-873 and Arizona Administrative Code R9-6-701 thru 708.  The law offers no grace period. You must provide proof of current immunizations in order to enroll your child in school.  If the student has a medical condition or personal belief that conflicts with this law a waiver may be signed and presented at the time of registration.  You may obtain the waiver at any of the Pima County Health Department immunization clinics.

The law does not allow parents or guardians to claim an exemption simply because the child's immunization record is lost or incomplete or because it is too much trouble to go to a physician or clinic.

 

Arizona School Immunization Requirements

Parents:

1. Children must have proof of all required immunizations, or valid exemption, in order to attend the firstday of school. Arizona law allows exemptions for medical reasons, laboratory evidence of immunity andpersonal beliefs. Exemption forms are available from schools and at www.azdhs.gov/phs/immun/idr_forms. Homeless students are allowed a 5-day grace period.

2. The record for each vaccine dose must include the date and name of doctor or clinic.

3. The statutes and rules governing school immunization requirements are:

Varicella (Chicken Pox) Immunization Requirement

Children who have not had chicken pox, and are attending child care, preschool, Head Start, kindergarten,and grades 1, 2, 3, 4, 5, 7, 8, 9, 10 and 11 during the 2009-2010 school year are required to show proof of varicella immunization. As displayed in the chart below, grades 6 and 12 will be added in 2010 when allgrades K-12 will be included in the requirement.

Schools and child care centers will accept a parent’s report that their child has had chicken pox. Children who have not had chicken pox need to be vaccinated. Parents/Guardians should contact their physician or local health department to arrange for their child to receive the vaccine. Children who receive varicella vaccine at 1-12 years of age need just one dose. Children who receive varicella vaccine beginning at 13+ years of age need two doses, spaced at least 4 weeks apart. Height and Weight screening

Screenings are completed by March. If you have any questions please feel free to call the health office at 731-4817.

Kindergarten

Please check with your physician regarding your child's immunization status.

Students entering kindergarten will need:

  • 5 DPT (Diptheria/Pertusis/Tetanus) - the last shot needs to be after the 4th birthday
  • 4 OPV/IPV (Polio) - the last dose after the 4th birthday
  • 2 MMR (Measles/Mumps/Rebella) - the 1st dose must be after the 1st birthday and the 2nd dose at least four months after
  • 3 HepB (Hepatitus B)

Note for 5th grade parents - You will be notified if your child is not up to date for middle school requirements

Immunizations can be obtained at no charge from the Pima County Health Department. Information about their locations and hour can be found on the Pima County Immunization Clinics website.
