NCAA Clearinghouse student release form: This form authorizes high schools to release student transcripts, including grades, proof of graduation, and other academic information to the Clearinghouse for Division I and II athletics. It also authorizes the Clearinghouse to release this information to the colleges that request it. The form can be filled out online.
