NOTE: The Health Office is closed the last hour of the school day.
Any student coming to the Health Office must have a pass from his/her teacher. Students may not come during passing period. The Health Office does not “fix” attendance, therefore, all students must check into class prior to visiting the nurse. ONLY EMERGENCIES WILL BE SEEN WITHOUT A PASS.
If you have a major health problem such as asthma, cancer, depression, diabetes, epilepsy or heart problem, please stop by the Health Office and let the nurse know. There may be forms to sign in order to properly care for you child during school hours.
If you need to bring medication to school, it must be in a prescription container and be kept in the Nurse's Office. Parents must sign the Permission to Administer Medication form for each medication, renewed each year. Please alert the Health Office if your child will be participating in a field trip and requires daily medication or nursing procedures.
Any student needing to go home due to an illness must make a phone call to a parent or guardian from the Health Office. Students that provide their own means of transportation (ie: city bus, vehicle, walk) must still have parent permission to leave campus (a school staff member must speak directly to a parent or guardian).
IMMUNIZATIONS: CHANGES FOR THE 2011-2012 SCHOOL YEAR: These changes are applicable to all incoming 9th grade students only. Students must show proof that they have received a current Tdap (tetanus booster) and MCV (Menningococcal/Menactra). All other students must show proof that they are current with their vaccinations. You will have 2 weeks from the "due" date to supply the proof of the immunization(s) required and PREVENT your son/daughter from being removed from school. Students who do not provide proof of immunization within a reasonable time will not be allowed to attend school until the proper documentation is recorded in the Health office. Parents must make this a priority.
PHYSICAL EXAMS: For all students planning to participate in the sports program, a physical examination must be done after March 1st of the current year for the upcoming school year and before the student can practice. The physical examination card must be signed by a licensed physician. Please turn in all completed forms to the Athletics Secretary.
