All students participating in AIA-Sanctioned athletics must have the following on file in the
Rincon/UHS Activities Office:
- Physical Exam Form
- Parent Permission Form
- Birth Certificate
- Emergency Card
- AIA Eligibility Questionnaire
- Receipt for Participation Fees
