The State of Arizona immunization laws require middle school students to be up to date in Hepatitis B (selibraryries of three shots) and have two MMR immunizations to begin the school year. If your child’s immunizations are not current, please make arrangements to get these injections now. All shots must be completed in time for the new school year.  Bring your child’s immunizations record with you to Registration, as schedules will not be issued without proof of immunizations.

Chicken Pox Vaccine will be required for students entering 7th and 8th grades if the child has not had the chicken pox disease.

 

Each student will need to have a current green emergency card, on file, filled out by parent or guardian.  Please list current health problems, medications and allergies in the appropriate section of the emergency card. This will assist the school nurse to give your child the best possible care, and if needed, be able to reach you in the case of an emergency.
