Question:
If the student withdraws before the 45th day, do you have to
complete the form and file it in the cum folder?
Answer:
You have 45 days to complete student screenings whenever the
child remains in the district for 45 days. If the student
withdraws before the 45th day, you do not need to complete the
screening.
Question:
If a student does not attend school and is automatically
withdrawn before the 45 day time period, but then returns to school and re-enrolls, is the 45
day timeline the original date, or does the time begin again
upon the re-enrollment?
Answer:
You have 45 days to complete student screenings whenever the
student remains in the district for 45 days. If a student drops
before the 45 day timeline is completed and then re-enrolls at a
later date, you will still have 45 days to complete the
screening.
The pre-populated form that you have printed from the Mojave
Report Generator will list the original 45 day deadline.
The person responsible for creating the report at the school
site will need to recalculate the dates. Please handwrite the
new enroll date and "file in cum" date on the form.

Question:
If a student was enrolled in TUSD, left the district and has
returned do we need to do a "New Student" report if one was done
when the student was previously enrolled?
Answer:
If a student already has a form, you do not need to do this
again. This is a one time occurrence for all students.
Question:
I'm reviewing some cum folders
and have found that some students don't have the new student
screening form. Even though they matriculated over a year ago
should we generate a form on them and have current teacher fill
out or not worry about it?
Answer:
For students who matriculated prior to May 2, 2005, we are
not required to go back and complete forms on those students as
we have already been found "out of compliance" on those
students. In the best interest of the students, we would
hope that the site staff would address any concerns and seek
appropriate follow-up for those students.
Question:
If a teacher should misplace
this report for a student, can we make up a blank one or what
should we do?
Answer:
In the Mojave Generator, you
can run a report for just one student.

Question:
I heard a rumor that there is
a calendar of dates that the New Student form is supposed to be
run, But I can't for the life of me find one. Is there one? Can
I get a copy?
Answer:

Question:
I trained staff on the screening
form during a professional development time, but not all staff
were present. I will be following up with those who were absent
the first time, but wondered if this training is for teachers
only???? or do all staff (aides, janitors) need to have the
information?
Answer:
The 45 Day New Student Screening process is for principals,
office staff, and all certified staff.
It would be helpful to have aides informed. It is not
necessary to train janitors or food service staff.
Question:
In Section A of the form, Mojave generates a teacher's name.
How was that determined? As a principal, can I cross out
that name and assign it to another teacher because the teacher
listed is a "Shop" teacher and I prefer to have a more
"academically-based" teacher complete the form.
Answer:
The Mojave Report Generator selects the teacher that is
listed as the homeroom teacher. As the principal, you may
cross out the name of that teacher and write in the name of
another General Education teacher (unless the student is in a
Special Ed self-contained classroom).
In Middle and High schools, the responsible teacher should
collaborate with the students other teachers to determine if
there are concerns.
Question:
Do we need
to complete this form for students who already have IEPs?
Answer:
Yes, please see #10 below.
Question:
For
new kids that come with an IEP, and where concerns are noted on
the 45-day screening form by the teacher and the form is
forwarded to the Administrator, would the Administrator mark
“other” and then note that the child is on an IEP?
Answer:
If the concerns are addressed in the IEP, then the
adminstrator would
mark “other” and then note
that the child is on an IEP.
If the concerns are NOT addressed in the IEP, then the
administrator would notify the parent within 10 school days from
the date the teacher returned the form and would document the
appropriate follow-up action taken.
Question:
Are we filling out this screening
form for new Pre-K kids like we do for new K-12 kids?
Answer:
Yes

I
have a student that did not show up on my report. She's a 6th
grader and new to TUSD.
Answer:
For students that you know are new to TUSD but who do not
show up on the report created by the Mojave Report Generator,
please contact the Help Desk at 225-6333 or

Question:
Is
there a form letter to be sent out after the 45-day screening
has been completed?
Answer:
Once the teacher returns the form - if there is one or
more “yes”, the principal must contact the parent within 10 days
by a form letter or documented phone call or documented face to
face meeting. Two templates for parent notification can be found

It is very important that whatever method is utilized the
principal document on the 45 day screening form the date the
parent was contacted. It is not required that a parent be
notified if there are no areas of concern.
