Roskruge Middle School Interscholastic Sports

Parents and Students:  Interested in participating in our Middle School athletics?  If you are interested in participating you need to turn in these four items before you practice:

·         One-time Physical Examination Form (Dated March 1, 2011 or later)

·         Interscholastic Emergency Information Card for each sport

·         Parent/Guardian Permission for Participation in Interscholastic Activities for each sport

·         Transportation release form for each sport

·         One-time $20 Activity Fee (Checks made out to TUSD/Roskruge)

           (You will receive a receipt and you may use the fee paid as part of your Arizona tax credit donation)

 

Without all of these items you will not be allowed to participate, this includes practice.  These forms are to be turned in to the coach or Mr. Dale López in Room 125.  All questions and concerns should be addressed to the coach, Mr. Dale López, or the administration.

TURN IN ALL THESE ITEMS AS SOON AS POSSIBLE. 

NO FORMS = NO PRACTICE!!!

First day of Practice for:

 Boys’ Basketball/Girls’ Volleyball:  August 23

Boys’ Volleyball/Girls’ Basketball: October 18

Boys’ and Girls’ Soccer: December 13

Boys’ and Girls’ Track and Field:  February 21
