Introduction

What’s in your family tree? Is your family predisposed to certain health risk? If so, what can you do to prevent similar health risk?

Tasks

1. Make a list of family members and grandparents and attempt to identify any health problems, and health assets associated with each person. Use this medical history form.   On the My Family Tree organizer  layout your family medical history (download the form, print it and write in the name of family members and their health risk).

2. Determine how you are at risk for health problems associated with your family using the resources available.

3. Formulate a prevention plan to reduce your risk of the health problem most prevalent in your family. Include a reference list of literature that supports your prevention plan.

4. Using the lifespan link, project any increase in lifespan you would anticipate as a result of following your prevention plan. Record and report this change at the bottom of your prevention plan.  

5. Take the information from your Health Risk Prevention Plan and use it to write your Prevention Plan essay..  Turn in the essay, the prevention plan,  and the notes from your organizer..  This should include your works consulted.
